Survivors include his mother, Verna Felix; brothers, Harold D. Cook, Daryl E. Wagner, and Paul D. Wagner; spouse, Cynthia Cook; children, Amanda Darocha, Joseph Cook, Nicole Brend, Ashley Conklin, and Megan Cook.
